Discussion: Courtesy Runner

Batter A gets on base and asks for a Courtesy Runner. Runner B becomes the runner and either scores or is out at another base. Later in that same inning, Runner B comes to bat, gets a hit and is on-base. He then asks for a courtesy runner.
Is runner B out? I think not bu cannot find wording to that effect. Some of umpires say he is out. Cab someone claify the ruling?
